At UConn Health, this position is primarily accountable for compliance with the institutions Commission on Cancer accreditation standards in community outreach and engagement (COE). The role provides community based lay navigation in health education, prevention, and early detection while reducing or eliminating barriers to care and improving access to services to underserved communities within UConn Healths catchment areas. THIS IS A TEMPORARY POSITION WITH NO BENEFITS SUPERVISION RECEIVED: Works under the general supervision of an employee of higher grade in the performance of routine tasks. Receives closer supervision on more advanced complex issues. EXAMPLES OF DUTIES: With the assistance of the program supervisor, performs an assessment of clients strengths and needs Works as part of the multidisciplinary team to prepare and implement plans Educates and counsels clients such as coordinating and referring medically underserved women to the CT WISEWOMAN and Connecticut Breast and Cervical Cancer Early Detection Program Assists clients in linking with community support systems and agencies, monitors client progress within the health/community support systems Provides education and training to clients in community living, activities of daily living skills Provides client outreach Completes all required documentation If specified, serves as a translator for the staff and parents Performs related duties as required. MINIMUM QUALIFICATIONS REQUIRED KNOWLEDGE, SKILL, AND ABILITY: Knowledge of community resources or providing supplementary assistance (as specified community outreach, underserved women) to clients/families Considerable interpersonal skills, oral and written communication skills Ability to work with a diverse client population EXPERIENCE AND TRAINING: General Experience: Four (4) years experience in the community working with human services or social services programs as a Community Health Worker including developing strategies to overcome barriers to care. SUBSTITUTIONS ALLOWED: College education in social work, psychology, or other related public health field may substitute for the general experience on the basis of fifteen (15) semester hours equaling one-half (1/2) year of experience to a maximum of four (4) years for a bachelors degree. Certification pertinent to program specialization can substitute for the general experience. SPECIAL REQUIREMENTS Employees in this class will be required to travel within the State in the course of their daily work. PREFERRED REQUIREMENTS: Experience working with diverse populations in a clinical setting Experienced in motivational interviewing techniques Ability to read, write English and Spanish Working knowledge of EPIC Proficient in the use of Microsoft Word, Excel, PowerPoint, and Outlook WORKING CONDITIONS: Incumbent in this class may be exposed to communicable diseases. WORK SCHEDULE: Part time, 24 hours per week, four days per week, six hours per day as assigned to accommodate program needs, may include evenings, weekends, and holidays. HOURLY RATE: $30.00
