HOM has a case management position available in both locations Portland and Augusta 26-35 hour/week. $35.00 hourly, plus paid training, meetings, supervision, travel time, and documentation.

Requirements:

18 years or older with bachelor’s degree

MHRT/C

Reliable transportation with insurance coverage

Ability to do a minimum of 26 billable hour weekly

Eligibility to work in the USA

The CM will assist and support adult clients who are diagnosed with a mental health diagnosis in developing coping skills, coordinating services in the community, building relationships such as natural supports, and ensuring follow through with necessary mental health and medical appointments. The CM supports the adult client in understanding better what the barriers are, breaking them down, and supporting them in building resiliency so they can manage such relationships in the community without support. The CM monitors progress and provides accurate and efficient documentation to ensure the clients needs are reflected.
